|Notice on the Launch of Electronic Health Declaration Form for Foreign Passengers|
Starting from 13 November,2020, all foreign passengers who are flying from Australia to China will be required to present the Electronic Health Declaration Form for boarding.
Foreign passengers with certificates of negative results of both nucleic acid and IgM antibody tests taken in NSW shall apply for the Electronic Health Declaration Form by logging on the website of http://hrhk.cs.mfa.gov.cn/H5/ via computer or smart phone, or by scanning the QR Code below.
Foreign passengers need to register, fill in the information, declare the health status and upload the testing certificates online, which will then be examined and verified by the Chinese Consulate-General in Sydney. Upon approval, applicants will receive the Electronic Health Declaration Form, which will be displayed in QR code with the "HDC" mark. Foreign passengers must present the electronic or printed QR code with the "HDC" mark, and follow the procedures for inspection by relevant airlines during boarding.
The Chinese Consulate-General in Sydney does not process the applications for "HDC" Mark by foreign passengers going to China via connecting flights at a third country.
